5 Mentorship Session Templates for High-Impact Programs

Most programs lose their impact data because they rely on vague session notes. We’ve designed these 5 templates to help you capture what actually matters - automatically.

If you’ve ever been a mentor on a mentorship programme, there’s a good chance you’ve been asked to fill in a generic box providing “feedback” after each mentorship session. And often you’ll find yourself asking - “Just what am I supposed to write here? Who is it for? Why do I need to do this?”. These generic response boxes can feel like such a burden for the mentor, and it can be really difficult to extract what progress is being made.

That’s why notes from a mentorship session should always have some structure to them - it helps the mentor understand what feeds into the overall goal of the programme, and gives them a clear level of expectation. It also helps when it comes to reporting - rather than comparing huge blocks of text, you can quickly pull out what’s important.

Below are five suggested mentorship notes templates, that you could adapt to your programme.

1. The Wellbeing Check-in

Goal: To check in on a mentee and make sure that they’re doing ok. We could all use this sometimes!

Questions:

  1. How is the mentee feeling today? (Sentiment score: 1 - 10)

  1. Energy Levels (Choose from High, Balanced, or Low)

  1. Do they have any concerns that require immediate escalation? (Checkbox)

  1. Support required (Long form notes on what the mentee needs help with)

2. The Internal Company Upskilling Log

Goal: To prove that the internal mentorship programme has been a useful addition to the company L&D strategy.

Questions:

  1. Skill focus: Multi select - eg Leadership, Technical, Soft Skills)

  1. Hours focused on upskilling: (Number of hours, trackable each week)

  1. Progress notes: (Long form notes that describe the progress of the mentee)

  1. Promotion readiness: Is the mentee on track for their next role? (Checkbox)

3. The Accelerator / Startup Growth Sprint

Goal: To move the needle on business metrics, and keep the startups focused on what matters.

Questions:

  1. Core KPI updates: (eg revenue, user growth, number of customers spoken to)

  1. Blockers: (What is the number one thing stopping progress this week?)

  1. Can anyone else help them?: IE would an introduction to a different mentor be useful? (List the name of the mentor)

  1. Investor readiness: (Score from 1 - 10)

4. The "Career Pivot" and Alumni Bridge

Goal: To help universities link their alumni network to existing students to boost their chances of finding a job.

Questions:

  1. Network Expansion: How many new industry contacts were made this week?

  1. Confidence Level: Self-reported score on job-market readiness.

  1. CV/Interview Milestones: Checkbox for completed tasks.

  1. Industry Placement Status: Single select: Seeking, Interviewing, or Placed.

5. The Volunteer & Community Support Tracker

Goal: To support volunteers at larger organisations - making sure they are getting value out of giving their time

Questions:

  1. Volunteer Satisfaction: How rewarded do they feel by their current role?

  1. Hours Logged: For reporting and grant compliance.

  1. Resource Gaps: Do they have the tools they need to succeed?

  1. Burnout Warning: A simple toggle for "High Stress" to alert the coordinator.
